Version: 3 October 2026. This placeholder is for local testing of online booking. It requires professional and legal review and approval by the practice before public use. Business terms that have not yet been agreed, including missed-appointment fees and payment deadlines, are not established by this draft.
1. Provider and scope
Direct booking is offered by Sustainable Med Berlin, Jan Okoye-Weeg, Schicklerstraße 5, 10179 Berlin. Contact: info@sustainable-med.com, +49 152 33520967. These terms describe online booking of an initial consultation and subsequent access to the patient portal.
2. Reservation and confirmation
Choose an available appointment and enter your contact details. The time is reserved for 30 minutes. Verify your email and confirm the appointment using your personal link within this period to complete the booking. Unconfirmed reservations are released automatically. All displayed times use the Europe/Berlin time zone.
3. Treatment and fees
Sustainable Med is a private practice. Medical services are billed under the German medical fee schedule (GOÄ). We explain planned services and expected costs before treatment. Reserving an appointment does not guarantee reimbursement by your insurer. Medical information, consent and treatment agreements are handled separately; online booking does not replace them.
4. Rescheduling and cancellation
Online booking and rescheduling currently require at least 48 hours’ notice. You can reschedule using your personal management link or patient portal while the existing appointment is also at least 48 hours away. Your current appointment remains booked until the change is saved successfully. Contact the practice by email or telephone for short-notice changes or cancellations. This draft does not establish a missed-appointment fee.
5. Your details and portal access
Please provide accurate and current contact and identifying details. Confirmation and management links are personal; do not forward them. After confirmation, use the patient portal to complete your medical history. Medical details belong in the portal, not the booking request. Booking is not medical advice and is not suitable for emergencies. Call 112 in a medical emergency.
6. Privacy and availability
Our privacy notice explains how your details are processed for appointment planning, confirmation emails and the patient portal. If technical problems arise, contact us by email or telephone, or use the alternative Doctolib booking link. Statutory patient and consumer rights remain unaffected.
